LPN Private Duty Charge in Athens **Critically Looking to fulfill day shift 7 days a week***
LPN needed to assist an adult who requires skilled nursing in order to live in his own home. Nurse team works alongside Direct Support Professionals who assist with non-skilled activities in both the home and the community.

Essential Responsibilities:
- Provide skilled nursing services according to healthcare plan
- Collaborate with other LPNs and work under the direction of agency RN
- Provide oversight of service delivery and documentation
- Conduct medication and eMAR reconciliations
- Track and manage medical appointments
- Monitor and report health and health risk data
- Act as a liaison and coordinate care with pharmacies, physicians, and others
- Maintain accurate and complete records
- Collaborate with Direct Support Professionals (DSPs)
- Personal Care Tasks. ISP goals (communication, social and educational activities, etc.)
Required Abilities
- Demonstrated ability to work collaboratively with others
- Communicate clearly both in writing and speaking
- Demonstrated ability to use electronic data systems and Microsoft Office: Word, Outlook, Excel
- Willingness to implement person-centered practices of agent qualifications
- Successful completion of a formal educational program in practical nursing approved by the Georgia board of nursing
- Current GA LPN license
- At least one year of home health, long term care, or acute nursing care experience
- Experience working with adults with disabilities preferred
