LPN Private Duty Charge in Athens needed to assist an adult who requires skilled nursing in order to live in his own home. Nurse team works alongside Direct Support Professionals who assist with non-skilled activities in both the home and the community.

Essential Responsibilities:

• Provide skilled nursing services according to healthcare plan
• Collaborate with other LPNs and work under the direction of agency RN
• Provide oversight of service delivery and documentation
• Conduct medication and eMAR reconciliations
• Track and manage medical appointments
• Monitor and report health and health risk data
• Act as a liaison and coordinate care with pharmacies, physicians, and others
• Maintain accurate and complete records
• Collaborate with Direct Support Professionals (DSPs)
• Personal Care Tasks. ISP goals (communication, social and educational activities, etc.)

Required Abilities

• Demonstrated ability to work collaboratively with others
• Communicate clearly both in writing and speaking
• Demonstrated ability to use electronic data systems and Microsoft Office: Word, Outlook, Excel
• Willingness to implement person-centered practices of agent qualifications
• Successful completion of a formal educational program in practical nursing approved by the Georgia board of nursing
• Current GA LPN license
• At least one year of home health, long term care, or acute nursing care experience
• Experience working with adults with disabilities preferred
