Job Overview
We are actively seeking full-time and part-time Skilled Nurses (LPNs and RNs) to join our dedicated team. In this vital role, you will deliver high-quality nursing care to individuals with diverse needs. Your expertise will help promote health, safety, and well-being for our clients across various care environments.
Duties
- Deliver specialized nursing care for individuals in home and community-based care settings.
- Provide comprehensive patient assessments, vital signs monitoring, and medication administration in accordance with healthcare protocols and regulations.
- Assist with medical documentation using electronic health records (EHR) systems to ensure accurate record-keeping.
- Support patients with complex needs including catheter care, tube feeding, and utilization of respiratory support equipment.
- Manage emergency situations to ensure patient safety.
- Work collaboratively with non-licensed care providers.
- Maintain compliance with private home care regulations and HIPAA confidentiality standards while providing compassionate patient-centered care.
- Collaborate with interdisciplinary teams on case management and individualized care plans tailored to each client’s requirements.
- Provide oversight and assessments (scope based on license credentials.)
Required/Preferred Experience
- Prior experience working in home environments is preferred, although facility-based long-term care, med-surg, ICU, emergency room, or trauma center experience will be considered.
- Background in working with individuals with developmental disabilities or behavioral health needs is preferred.
- Proficiency with EMR/EHR systems for documentation and patient tracking.
- Knowledge of ICD coding (ICD-9/10), medication administration protocols, and diagnostic procedures enhances your effectiveness in this role.
- Strong understanding of physiology, anatomy, pharmacology, and pain management principles to support optimal patient outcomes.
